Please bring Player Registration Form:

1. Open Player Registration form;
2. Type in information;
3. Print out form – SIGN at bottom and Initial in Box;
4. Sign form and make 1 copies (2 total)

New Foothill Storm Players: Original Birth Certificate. (For Returning Players a copy of Birth Certificate is ok.)
All Players 2 head shot pictures, postage stamp size.

Cal South forms:
